• Visit https://mylogins.edc.cingular.net/login.cfm and login
• Click the Login Request Form link
• Select Account Type = CUID, Organization = Information Technology, Department = UNIX OS Level Access Other
• Select Application = UNIX OS Other
• Select Request Type = Add
• Press the Next button
• Enter your User CUID and press Search
• Click Next

For the rest, you'll have to answer questions about which server and what kind of access you want.   The process seems to work pretty well.  It took me 2-3 days to get my non-expedited access.
